Lana

I am a long time dog owner, currently mom to my 14 year old golden retriever. I’m experienced with many breeds and senior care or specific needs. My number one priority is providing your furry babies with a home away from home.

I have a large fenced yard and a senior Golden who has full reign of the home. There are also many parks and trails for walks in my suburban neighbourhood.

Playtime and basic obedience are practiced while socializing with other pets. I will provide regular exercise and constant play time supervision of your pet. Play time may consist of outdoor play, often with other animals. Off-leash socialization for dogs, where permitted, is encouraged.

I accept dogs of all ages and breeds. Please let me know if your dog has any allergies, dietary restrictions, special needs or behaviours that I should be aware of.

I only accept friendly and well behaved dogs as I strive to provide a safe and healthy environment for your pet and others during their stay. I accept a maximum of 4 dogs at the same time.

Waiver and Release – deemed as agreed when you confirm your booking:

“I hereby declare that I am the legal owner of my dog and that my dog has not been exposed to rabies, distemper, parvovirus, parainfluenza or any other contagious disease within the past thirty (30) days and that my dog has been up-to-date vaccinated. (Please provide proof of current vaccinations for our records).

My dog’s boarding is not without risk. It is in a home care practice not necessarily with 24 hours human watching. I authorize Lana Chan (“Lana”) to do whatever she deems necessary for the safety, health, and well-being of my dog while under the care of Lana including seeking professional treatment for my dog.

(Every effort will be made to provide a safe environment for your dog. If your dog requires medical attention, we will contact you immediately and make arrangements with you as to where to take your dog. If we cannot reach you, we will transport your dog to an Animal Emergency Clinic)

I am solely responsible for any damage or injury incurred while my dog is boarded, including payment of costs of injury to caregiver, other clients, and other animals or damage to facilities caused by my dog. I waive any and all claims, actions or demands of any nature, foreseen or unforeseen, that I may have against Lana relating to the care, control, health and/or safety of my dog arising during the pick-up, transportation, drop-off and stay at the facilities.

I understand my dog’s acceptance into boarding is discretionary and that Lana has the right to refuse service to me and/or my dog at any time for any reason. I understand that if my dog has a history of or repeatedly demonstrates aggression or biting humans or animals, Lana reserves the right to refuse service.

I, upon booking confirmation, agree to the above, accept full responsibility for the acts of my dog while being boarded and release Lana and its caregivers of all claims of liability.

I acknowledge that I have carefully read all of the above, including this Waiver and Release, understand its contents, and understand that this agreement includes an assumption of the risk of Lana’s negligence and liability. I acknowledge that Lana is materially relying on this waiver and is allowing my dog(s) to engage in play with other dogs in the daycare and boarding, and the facility during their stay in Lana’s boarding facility.
